ConnectRx Wisconsin is part of the Saving Our Babies initiative led by the Dane County Health Council, the Foundation for Black Women’s Wellness, and community partners. The initiative was created to improve Black maternal and child health outcomes and reduce low birth weight and infant mortality disparities in Dane County. ConnectRx links healthcare systems and community partners so Black pregnant and birthing people, babies, and families can be connected to care coordination, referrals, and community resources that address health, social, and family needs.
